On April 28th, 1988, the plane suffered a catastrophic structural failure, with a large portion of the upper canopy of the plane ripping off in mid flight. A flight attendant was killed instantly and the terrified passengers stayed strapped into their seats, as the plane made an emergency landing on Maui.
