He realizes that everything he sees and is perceives only takes place in his mind. His entire treatment is nothing more than an experimental method for finding the truth through manipulation of perception and reality. In the end it turns out that his wife and daughter were murdered 6 years ago and he was the only person in the apartment when the police arrived.
