Toni leads Lorraine to the staff lounge to wait while the hospital is searched. Althea and Nick continue to declare their love for one another, but Nick flatly refuses Althea's advice to marry Cathy. He reiterates that John is trying every trick in the book to worm his way into her life. Mike spots Toni and Vito together in the elevator, and just as the doors close on them, Toni warns him that Al has a gun. When Althea goes home, John tries a new approach, lamenting the fact that he's half a man, an allusion to his sexual impotence. His despair moves Althea, who attempts to allay his fears for recovery. Searching the hospital basement, Mike finds a puppet Mary Nell has dropped, leading him into a linen supply room where she and Al are hiding. An increasingly erratic Al pulls his gun on Mike, who remains calm as he attempts to talk the man down. Mike coolly offers to take Mary Nell's place as Al's hostage. He will leave the hospital with Al and stay with him until they're across state-lines, going anywhere Al wants to go. Mike begins to move slowly away, his back to Al, but then he turns suddenly and lunges for the gun. It fires, and Mike falls to the floor. Al takes aim again to finish the job, but before he can shoot, Vito bursts through the door and spoils the next shot. As Vito struggles with Al, Toni rushes to a wounded Mike's side.
